Please note that the attached form is in a PDF format with fillable facility so that each faculty can fill the information in the form in addition to upload his/her personal photo then the form can be saved and submitted.

If it is not possible to upload the Photo in the form due to using a different PDF reader then the faculty can submit the form and photo individually to the above email.
